Job Title: Program Coordinator
Home Office: Johnson County Office (JCO)
Area of Focus: Behavioral Health
Program(s): PATH - Projects for Assistance in Transition from Homelessness helps adults experiencing homelessness or marginal housing and those living with significant mental illness transition to more stable housing by coordinating services that help them address their individual needs. ReLeaf - Is a transitional housing program providing trauma-informed, no-cost housing and wraparound support services for survivors of domestic violence, sexual assault, human trafficking, and community violence across Southern Illinois. This program fills a critical regional gap by offering safe, flexible housing, and individualized case management to help survivors achieve long-term stability.
Immediate Supervisor: Director of Community Transitions - Daniel Hyson
Supervises: ReLeaf Case Manager, Residential Specialist
Hours of Work: Full-time 40 hours/ week. Participates in after-hours 24/7 crisis rotation.
FLSA Classification: Non-exempt
Program Coordinator Minimum Qualifications: Bachelor’s Degree in Human Services or related field, qualifies as a Mental Health Professional (MHP), with at least 5 years of experience.
Program Coordinator Special Requirements: Must be 21 years of age or older to drive agency vehicle; maintains a valid driver’s license and vehicle liability insurance; operates agency vehicles and personal vehicles while driving for agency business in a safe and lawful manner at all times; maintain First Aid & CPR certification; must complete required registry clearances and criminal fingerprint background checks; schedules evening or weekend hours, as necessary; participates in after-hours 24/7 crisis rotation.
Program Coordinator Skills & Competencies: Employee must demonstrate strong written and verbal communication skills, active listening, and the ability to work effectively with individuals experiencing trauma, homelessness, or victimization. Knowledge of trauma-informed care, crisis intervention, housing stabilization practices, and case management processes is required. The employee must possess knowledge of community resource coordination, housing navigation, victim advocacy, and supportive service delivery models. Experience working with individuals experiencing mental illness, substance use disorders, or domestic violence is preferred. Employee must have knowledge of Medicaid Administrative Rule 132, housing program standards, and applicable federal and state grant requirements. Familiarity with VOCA guidelines, PATH program requirements, and CARF accreditation standards is preferred. The employee must demonstrate strong leadership and supervisory skills and be able to support staff development and performance improvement. Strong organizational skills, computer proficiency, and experience with electronic health record systems are required.
Program Coordinator job duties include, but not limited to:
- Oversees daily operations of the PATH Outreach Program and Project ReLeaf Transitional Housing Program, as directed.
- Will service as primary contact for ICJIA regarding program implementation, compliance, reporting, and grant monitoring activities.
- Coordinates referrals, intake screening, and program eligibility for individuals served through PATH and ReLeaf.
- Intervenes and evaluates clients, establishes diagnosis, as needed. Assists in the development of the individual treatment plan collaboratively with clients and the QMHP by defining goals, specific outcomes and time frames for treatment. Monitor client's behavior.
- Conducts crisis intervention to stabilize client's condition by utilizing short-term crisis intervention techniques and/or referral to other support services.
- Supports program staff in providing trauma-informed case management, housing stabilization services, and resource coordination, as needed.
- Assists in ensuring clients receive individualized service planning, safety planning, and linkage to behavioral health, legal, employment, and community resources.
- Coordinates housing placement and stabilization for ReLeaf clients utilizing communal, clustered, and scattered-site housing options.
- Assists in ensuring clients housing units meet safety, habitability, and program standards and coordinates with agency maintenance staff for inspections and repairs.
- Maintains and develops collaborative relationships with internal and external partners including law enforcement, courts, victim service agencies, housing providers, advocacy organizations, and community partners.
- Ensures services are delivered in a trauma-informed, strengths-based manner consistent with victim rights, confidentiality standards, and agency policies.
- Provides direct services to program clients when necessary to maintain service continuity; ensuring documentation is completed timely and efficiently.
- Ensures transportation coordination and resource access for program clients when appropriate.
- Assists staff in identifying barriers to housing stability and developing solutions to support long-term independence for assigned clients.
- Participates in local coalitions and community meetings addressing homelessness, housing stability, and victim services, as applicable.
- Supports outreach efforts to ensure individuals experiencing homelessness or victimization are aware of available services.
- Ensures program compliance with federal, state, and grant-specific requirements including PATH and VOCA program standards.
- Oversees program data tracking, service documentation, and outcome reporting in the agency’s electronic health record system.
- Maintains accurate records and documentation to support compliance, audits, and quality improvement activities.
- Oversees the Arrowleaf Community Closet (Molina Closet) including tracking and organization of items, spending, and MATCH, if applicable.
- Provides supervision to PATH outreach staff, the ReLeaf Case Manager, and the ReLeaf Residential Specialist.
- Monitors staff productivity, documentation accuracy, and service requirements.
