Ark Healthcare and Rehabilitation at Branford Hills, a premier 190-bed Skilled Nursing Facility seeks a Full-Time Social Worker to join our team!
As a member of the Social Work Department you will play an integral role in enhancing the well-being of our residents and help them meet their basic and complex needs.
Responsibilities:
- Develop social, psychosocial history, formulate care plan which addresses problems, needs, and concerns.
- Participate in Interdisciplinary Meetings
- Maintain compliance with MDS assessments, care plan review and revision.
- Maintain compliance with federal and state documentation requirements .
- Inform, educate, and advocate on behalf of patients and their resident rights.
- Coordinate discharge planning, and assess patients for the potential of discharging.
- Provide counseling to residents
- Maintain compliance with required state assessment schedule, e.g. Assessment Pro Level I, IIs, Level of Cares and communicate changes or pertinent information to IDT.
- Serve as point of contact with hospice as well communal agencies and care providers for post-discharge environments.
- State/Federal Survey experience
- Participate in Quality Assurance and Performance Improvement Initiatives (QAPI) Program.
- Communicate with Probate Court in matters related to Conservatorships
Other Duties as assigned.
Skills:
Must possess a BSW, MSW, or have qualifications to serve as a designee detailed in CT DPH §19-13-D8t Public Health Code.
Must possess working knowledge of Healthcare /SNF/LTC Social Work practices
Must possess strong work ethic, communication skills, and writing skills.
