- Provide psychiatric case management services to adults
- Deliver individual and group interventions
- Assist clients in developing communication, relationship, and coping skills
- Provide crisis management and support as needed
- Complete assessments and assist in developing service plans
- Link clients to community resources and services
- Advocate for clients and support access to needed services
- Monitor progress and provide ongoing support
- Deliver services in homes, schools, and community settings
- Maintain timely and accurate documentation in compliance with agency standards
- Participate in team meetings to support a collaborative care approach
- Experience working with adults, families, and/or groups with behavioral health needs
- Experience with or understanding of:
- Trauma-Informed Care
- Crisis Intervention
- Family Systems approaches
- Strong organizational, time management, and documentation skills
- Ability to work independently in community-based settings
- Ability to build rapport with a wide range of individuals
- Knowledge of or familiarity with community resources (Ashland County preferred)
- Strong commitment to client rights and ethical practice
- Bachelor’s degree in a related field
- Experience in case management or behavioral health
- High School Diploma with relevant experience
- Valid driver’s license, reliable vehicle, and active auto insurance
- Ability to travel locally and provide services in the community
- Successful completion of background check, driving record check, and drug screening
