Role: Registered Nurse (RN) Home Health Case Manager, Hospice
Responsibilities include: providing compassionate nursing care to clients with terminal diagnoses, developing and implementing patient/family-centered care plans, guiding healthcare decisions, educating patients and families, consulting with physicians, coordinating hospice care with team members, conducting assessments, and evaluating patient conditions through informational visits.
Qualifications and Skills: Missouri or Compact RN license, valid driver’s license with auto insurance, strong organizational, interpersonal, and managerial skills, high energy, motivation, and the ability to provide effective support in demanding situations.
Key Duties: work with interdisciplinary teams, ensure needs are met, guide healthcare decisions, and promote quality of life for hospice patients.
