Position: Registered Nurse (RN) Home Health Case Manager, Hospice
- Responsibilities: Provide compassionate hospice nursing care, educate patients and families on hospice processes, guide goals-of-care discussions, conduct assessments, perform evaluations, collaborate with interdisciplinary teams, coordinate care, and communicate patient condition updates.
- Qualifications: Active Missouri or Compact RN license, hospice or long-term care nursing experience preferred, strong assessment and communication skills, valid driver’s license, and auto insurance.
