TCOMI Intensive Case Manager

We are seeking a dedicated Liaison to serve as a key connection between Van Zandt County probation offices and individuals with mental illness. This role supports 20-25 probationers by coordinating mental health and community services, providing crisis intervention, and educating both clients and criminal justice partners.

Key Responsibilities

• Act as a liaison between Van Zandt County probation offices and assigned probationers with mental illness
• Identify, assess, and link clients to Andrews Center and other appropriate community services
• Provide crisis intervention and ongoing monitoring at clients' homes and other community locations as needed
• Educate criminal justice agencies and clients on Mental Health (MH) and Intellectual and Developmental Disabilities (IDD) treatment and available services
• Maintain accurate documentation and reports in accordance with program and agency requirements
• Travel extensively throughout the service area to meet client and program needs
  
  

Qualifications

• Bachelor's degree in a human services field required
• Experience in mental health services preferred
• Experience working with the criminal justice system preferred
• Strong communication, assessment, and crisis intervention skills
• Ability to work independently in community-based settings
• Valid driver's license and ability to travel extensively
