Position Summary
The Executive Director will serve as the senior operational and clinical leader of CABS Health Network, overseeing the Home Care, Care Management, and Social Care Network divisions.
Reporting directly to the CEO, this role will provide strategic and administrative leadership to ensure all programs operate efficiently, compliantly, and with the highest standards of quality and compassion.
The Executive Director will also drive program innovation and partnership development, strengthening CABS’ role within New York’s integrated care ecosystem and expanding our capacity to address the full spectrum of medical, social, and environmental health needs.
Key Responsibilities
- Lead day-to-day operations for Home Care, Care Management, and Social Care Network programs.
- Supervise senior leadership, including the Associate Directors of Care Management, Nursing, and Social Programs, and the Assistant Director of Case Management.
- Translate the CEO’s strategic vision into actionable goals, measurable outcomes, and department-level accountability.
- Build and sustain partnerships with hospitals, managed care plans, IPAs, and community-based organizations.
- Oversee compliance with state, federal, and funder requirements, including DOH and Medicaid Managed Care regulations.
- Develop and implement new programs, pilots, and funding initiatives that expand access and improve client outcomes.
- Ensure clinical quality, operational efficiency, and data-driven decision-making across all divisions.
- Mentor senior managers and foster a culture of accountability, collaboration, and excellence.
Qualifications
- Education:
- MD, Nurse Practitioner (NP), or Master of Science in Nursing (MSN) — required.
- Master’s in Business Administration (MBA) or Public Administration (MPA) — required.
- Experience:
- At least 10 years of progressive leadership in healthcare, care management, or community-based program administration.
- Proven experience in acute, post-acute, and community-based care.
- Demonstrated success leading multi-disciplinary teams and developing cross-sector partnerships.
- Deep understanding of Medicaid Managed Care, Social Care Networks, and value-based payment models.
- Skills:
- Strong operational leadership, communication, and team-building skills.
- Expertise in program development, compliance, and quality improvement.
- Ability to balance strategic vision with hands-on execution.

Pay: $145,000.00 - $150,000.00 per year
