Job Summary:

The LVN in a Skilled Nursing Facility (SNF) plays a crucial role in providing compassionate and skilled nursing care to residents. They work under the supervision of registered nurses and physicians to ensure the safety and well-being of patients.

Responsibilities and Duties:

1. Administer medications and treatments as prescribed by healthcare providers.

2. Monitor and record vital signs and patient progress.

3. Assist with activities of daily living, such as bathing, dressing, and eating.

4. Communicate effectively with patients, families, and healthcare team members.

5. Keep accurate and detailed medical records.

6. Collaborate with other healthcare professionals to create individualized care plans.

7. Educate patients and families about healthcare needs and treatments.

8. Ensure compliance with all relevant laws, regulations, and policies.

Qualifications and Skills:

1. Valid LVN license in the state of practice.

2. Previous experience in a skilled nursing facility or long-term care setting preferred.
