Description
American Traveler is seeking an experienced RN for a Home Health/Hospice position serving a primarily geriatric population, requiring a WI or compact RN license and at least 1 year of experience.
Details
- Post-acute home health and hospice care setting — no hospital unit, no floating between departments, but clinicians will perform both home health and hospice visits
- Primarily geriatric patient population with diagnoses including heart disease, CHF, COPD, pneumonia, dementia/Alzheimer's, depression, stroke, cancer, kidney disease, diabetes, liver disease, wounds, joint replacement, and post-surgical recovery
- No pediatric patients
- Caseload of 5–6 patient visits per day, approximately 25–30 points per week, including routine and OASIS visits
- No ventilator-dependent patients; CPAP and BiPAP are used on occasion and self-managed by patients
- Charting system: Cerner (transitioning to Epic)
- Monday–Friday, 8:00am–4:30pm (end time may vary); 8-hour shifts
- Weekends required only during on-call rotation
- On-call rotation of 1 week/weekend every 6 weeks; on-call days do not need to be consecutive
- At least 1 major and 1 minor holiday coverage required
- 13-week contract
Requirements
- Active WI or compact RN license required by start date
- Current BLS certification (AHA) required
- Minimum 1 year of RN experience required
- Prior home health and/or hospice experience strongly preferred; travel nursing experience preferred
- Required clinical skills: chronic disease management, patient/family education, NPWT, ostomy care, central line care, IV administration, and complex wound care
- Hospice-specific skills: pain and symptom management, end-of-life care, empathy, and compassion
- Strong critical thinking abilities required
- 2 professional references required for consideration: 1 supervisor reference from within the last 12 months of relevant experience, and 1 peer or supervisor reference from the same specialty within the last 3 years
Additional Information
- Daily 15-minute huddle call, weekly interdisciplinary team (IDT) meeting at the office, and monthly all-staff meetings are part of the regular schedule
- Team includes 3 certified nursing assistants (CNAs); administrator is available after hours, on weekends, and holidays
- Orientation consists of 1 day of computer-based training followed by 2–5 days of field training and shadowing, depending on experience
- Scrub dress code: solid color scrubs only (no patterns or logos unless company-issued); closed-toe, non-slip shoes with socks or hosiery required — no open-toe shoes, sandals, or flip-flops
- Candidates who previously worked for this health system as permanent staff must have an end date at least 6 months prior to their start date
- Candidates residing within 80 miles of the facility are considered local and may be subject to a reduced travel pay rate
