Description
American Traveler is seeking an experienced RN House Supervisor for a 13-week travel assignment at a skilled nursing facility in Rochester, NY, requiring prior charge experience and an active NY RN license.
Details
- Skilled nursing facility (SNF) setting with an average daily census of 180 residents across 5 distinct units
- Units include a neurobehavioral dementia/Alzheimer's unit (with residents exhibiting violent behaviors), a secure dementia/elopement-risk unit, a short-term rehab unit (post orthopedic surgery and cardiac procedures), and two long-term care units
- Resident diagnoses include dementia, Alzheimer's, DM, CVA, cardiac conditions, and orthopedic post-op
- House Supervisor role covers all units facility-wide, primarily during evenings, nights, weekends, and holidays
- Schedule consists of three 8-hour shifts and one 12-hour shift per week (36 hours/week total), with shift times ranging between 11p–7a, 3p–11p, and 7p–7a
- Every other weekend coverage required
- Special equipment includes wound vacs, PICC lines, nephrostomy tubes, tube feeds, indwelling catheters, and colostomies
- Epic EMR preferred
Requirements
- Active NY RN license required
- Current BLS certification required
- Minimum 2 years of RN experience required
- Prior charge or supervisory nursing experience required
- SNF experience required
- Proficiency in IV insertion and maintenance, PICC line care/management, and wound vac management
- Skills required include code management, de-escalation techniques, managing assaultive behavior, emergency preparedness, centralized staffing, and incident/sentinel event reporting
- Knowledge of regulatory requirements (DPH, TJC, OSHA, FMLA) and federal/state scope of practice labor laws
- 2 professional supervisor references from within the past 2 years required for consideration
- Full 7-year work history with all gaps explained required for consideration
- Must reside more than 75 miles from the facility to be eligible

- Serves as the facility-wide clinical resource and problem-solver during off-shifts, providing expertise, staff mentoring and coaching, and facilitating patient flow across all 5 units
- Responsibilities include supervising nursing care delivery across all units, new admissions, staffing coordination, coaching staff to performance standards, and managing workplace safety and infection control
- Holiday coverage is expected as part of the off-shift supervisor role
