Description
American Traveler is hiring an experienced RN for a Case Management/Care Coordination position at a Level II Trauma acute care hospital, requiring prior CA Dignity Health experience and proficiency in both discharge planning and utilization review.
Details
- Acute care hospital setting at a Level II Trauma facility
- Care Coordination/Case Management unit covering all inpatient areas of the hospital (no outpatient or home care)
- Handles a caseload of 15–20 patients/consults per day
- Covers all payer types including uninsured and homeless populations
- Role encompasses both discharge planning and utilization management (UM)
- EMR: Cerner; InterQual (IQ) criteria used for utilization review
- 8-hour day shifts, with hours between 8am–5pm
- Every other weekend required (at minimum one full weekend per month)
- First to float policy applies
Requirements
- Active CA RN license required (must be active at time of consideration; pending licenses not accepted)
- BSN required
- Minimum 2 years of RN experience required
- Prior recent CA Dignity Health experience is required
- BLS certification required (active; pending not accepted)
- Experience with InterQual (IQ) criteria required
- Proficiency in Cerner EMR required
- Strong skills in collaboration, teamwork, effective communication, and ability to reprioritize throughout the day
- Two professional references required: one supervisor from within the last 12 months worked, and one peer or supervisor from within the last 3 years worked (both must include dates of employment and eligibility for rehire)
- Valid driver's license required
Additional Information
- Responsibilities include completing initial assessments, InterQual reviews, discharge planning, and utilization management across all payer types
- Orientation includes 5 days of care coordination new employee orientation; unit orientation length is determined on a case-by-case basis
- Training on InterQual will be provided
- On-site support includes hospitalist/intensivist in-house and respiratory therapy (RT) available 24/7
- Holiday coverage is not required but candidates willing to work holidays are welcome
- Business casual attire required (no scrubs)
- Candidates who have been directly employed (full-time, part-time, or PRN) by any CommonSpirit, CHI, or Dignity facility within the past year are not eligible; candidates employed PRN through outside staffing firms will be considered
- RNs must pass the Relias Dysrhythmia exam before an offer can be confirmed
- A pass/fail skills test on sterile technique for Foley catheter insertion and central line dressing changes is administered on the first day of orientation
