American Traveler is seeking a Physical Therapist with a minimum of 3 years of experience for a 13-week contract in a long-term care/skilled nursing facility setting.
Details- Day shift schedule
- Work setting is a long-term care/skilled nursing facility
- Responsibilities span evaluation, treatment planning, and documentation across a resident population
- Electronic Medical Record used for all documentation
- Minimum 3 years of physical therapy experience required
- Bachelor's degree required; Master's or Doctorate preferred
- Active PT license in the applicable state required; license required if applicable
- BLS/BCLS certification required
- Prior experience in a long-term care or skilled nursing facility preferred
- Proficiency in Microsoft Office required
- Strong verbal, written, and interpersonal communication skills required
- Ability to lift up to 50 lbs independently; lifts over 50 lbs require a two-person assist
- Must be able to sit, stand, walk, bend, stoop, twist, and reach as part of daily duties
- DOB and SSN may be required for consideration
- Responsibilities include evaluating residents, establishing individualized plans of care with short- and long-term goals, and progressing treatment toward functional outcomes
- Collaborates with an interdisciplinary team including physicians, nursing staff, and other therapy professionals, including PT Assistants
- Provides education to residents and caregivers and documents teaching outcomes
- Participates in team meetings and consults with referring physicians on care plans and discharge needs
- Assists with new program development and provides in-service education to nursing and other departments
- Provides mentorship to new staff during orientation
- Responsible for maintaining department cleanliness and adhering to infection control procedures
- Must maintain all required licenses, certifications, and annual compliance requirements
