American Traveler is seeking a licensed Physical Therapist with 2 years of experience for a 13-week home health assignment in the Santa Maria, CA area, requiring OASIS documentation experience and Cerner proficiency.

• Home health setting providing patient care across assigned geographical regions
• Interdisciplinary rehab team includes PT, OT, and Speech Therapy, supported by rehab aides, OTAs, and PTAs (primarily permanent staff)
• Common diagnoses include orthopedic conditions, joint replacements, strokes, and spinal injuries
• Uses Cerner EMR
• Majority of work hours are Monday–Friday, 8am–5pm with 8-hour day shifts
• Every 3rd weekend rotation required
• Coverage expected for minor holidays; major holidays only as needed
• 13-week contract

• Minimum 2 years of PT experience required
• Active state PT license required at time of consideration; pending licenses not accepted — license required if applicable
• Current BLS certification required; pending certifications not accepted
• Experience with OASIS documentation required
• Cerner EMR experience preferred
• Home health setting experience preferred but not required
• Valid driver's license required
• 2 professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, including dates of employment and eligibility for rehire

• Responsibilities include evaluation and treatment of patients across an assigned geographical region in a home-based care model
• 1.5–2 week on-site orientation provided
• Business attire with a white lab coat required; jeans are not permitted
• Candidates who have been directly employed (full-time, part-time, or PRN) by any CommonSpirit or Dignity Health facility within the past year are not eligible; candidates previously employed PRN through outside staffing firms will be considered
