Director of Patient Services for Home Health Care
Located in Brooklyn, NY
Salary Range - $150K to $200K Per Year *based on experience*
Essential Duties & Responsibilities:
Maintain full compliance with NYS DOH regulations, audits, and LHCSA reporting.
Direct clinical workflows, including patient assessments, care planning, and documentation.
Supervise RNs, LPNs, and field clinicians; oversee training and credentialing of aides.
Monitor quality of care and implement corrective actions as needed.
Collaborate with managed care organizations to coordinate care.
Provide leadership in patient satisfaction and quality improvement initiatives.
Qualifications:
Valid NY RN license required.
Bachelor’s degree in Nursing; Master’s preferred.
7 years of nursing leadership experience in home care or related setting.
Proficiency in EMR and compliance systems.
