Position Overview
The Director of Patient Services (DPS) will play
a pivotal role in both clinical oversight and business growth. This
executive-level role requires a dynamic leader who can enhance clinical
outcomes, foster strong relationships with healthcare professionals, and
contribute to the expansion of our client base. The DPS will oversee clinical
operations, guide the care delivery process, ensure compliance with industry
standards, and work closely with the leadership team to drive growth. The DPS will work collaboratively with caregivers, clients, and families
to maintain a patient-centered approach while fostering a culture of excellence
and continuous improvement. The Registered
Nurse DPS will also actively participate in assessing client needs,
developing care plans, and providing medical expertise to ensure the highest
standard of care is delivered across all services.
Key Responsibilities
Client Retention and Satisfaction:
- Develop and implement strategies to
improve client retention through enhanced care coordination and communication
with families and clients.
- Lead initiatives focused on improving client
satisfaction, addressing client concerns, and optimizing care delivery.
- Review client feedback and implement
improvements to ensure continuous improvement in care quality and service
delivery.
Compliance and Quality Assurance:
- Ensure compliance with all local,
state, and federal regulations, including maintaining accreditation standards.
- Regularly review and update clinical
policies and procedures to ensure they reflect best practices and regulatory
requirements.
- Lead quality assurance and performance
improvement initiatives to enhance overall clinical operations.
- Conduct regular audits to ensure
clinical documentation and care practices are in line with industry standards.
Clinical Leadership:
- Oversee and manage all clinical
aspects of the agency, including assessments, care planning, and execution of
services.
- Ensure that the highest quality of
care is delivered to clients in accordance with industry standards and
regulations.
- Develop and implement clinical
protocols and best practices to improve client outcomes and satisfaction.
- Monitor and assess clinical
performance metrics, including patient outcomes, client retention, and overall
service quality.
- Provide leadership and support to
clinical staff, including RNs, LPNs, HHA’s, PCA’s and caregivers, ensuring
ongoing education and compliance with standards.
Business Growth and Relationship
Building:
- Actively engage with hospitals,
physicians, rehabilitation centers, nursing homes, and other healthcare
facilities to build strong referral relationships.
- Work with the marketing and business
development teams to create strategies that promote Synergy HomeCare’s services
to healthcare professionals and the community.
- Conduct presentations and outreach to
healthcare partners to increase awareness of Synergy HomeCare’s services.
- Collaborate with leadership to develop
strategic growth initiatives aimed at increasing census and expanding the
agency’s reach.
Team Leadership and Development:
- Mentor and lead clinical staff,
fostering a culture of excellence and professional growth.
- Support the recruitment, hiring, and
training of clinical staff to maintain a high standard of care.
- Organize and conduct ongoing
professional development and training for the clinical team.
Qualifications:
- Current Registered Nurse (RN) license in good standing (BSN/MSN)
- Minimum (2) two to (7) seven years of verifiable clinical and
managerial experience within a Home Care setting
- Cheerful
personality and good communications skills (bilingual
in Spanish and English a plus).
- Willing
to meet with and speak with patients, their family, and their medical professionals
to develop, update and maintain proper care plan.
- Experience with electronic health records (EHR) and care
management systems is preferred.
- Strong Knowledge of home care operations, clinical protocols,
and healthcare regulations in accordance with NYS DOH guidelines.
- Prior supervisory experience with field staff, and experience
with clinical oversight, monitoring quality assurance, and overall case
management
- Demonstrated ability to lead, mentor, and manage a clinical
team.
- Strong communication and PC
skills
- Skilled in identifying issues, developing
solutions, and implementing corrective actions to improve clinical operations.
