About the Role: We are seeking a highly organized and empathetic Home Health Intake Coordinator to serve as the critical first point of contact for our agency. You will bridge the gap between hospital discharge planners, physicians, patients, and our clinical team. The ideal candidate thrives in a fast-paced environment, possesses strong emotional intelligence, and is highly proficient with digital workflow and secure communication systems.

Key Responsibilities:

  • Receive and process patient referrals from hospitals, physician offices, and community facilities promptly and accurately.
  • Verify patient insurance coverage, Medicare eligibility, and secure initial authorizations for home health services.
  • Coordinate seamlessly with the Director of Nursing, Case Manager and appropriate clinicians to new cases.
  • Maintain highly accurate patient records within the agency’s EMR system.
  • Manage secure communications, documentation, and digital file handling using our agency's platforms.
  • Communicate clearly and compassionately with patients and their families regarding their benefits, out-of-pocket costs, and the start-of-care process.

Qualifications:

  • 1–3 years of experience in home health intake, medical admissions, or a closely related healthcare setting.
  • Excellent technical fluency; comfortable navigating EMRs, Google Workspace, and adapting to secure cloud environments.
  • Exceptional verbal and written communication skills with a customer-service mindset.

Salary : $19 - $22
