Core Responsibilities
- Patient Assessment and Care Planning:
- Assess patient health, develop and implement nursing care plans, and monitor patient recovery.
Direct Patient Care:
Administer medications, provide wound care, monitor vital signs, and assist with therapies and medical equipment.
Patient and Family Education:
Teach patients and their caregivers about their conditions, medications, and how to manage their health at home to improve outcomes.
Collaboration and Communication:
Work with physicians and other healthcare professionals to coordinate care, and report on patient progress.
Documentation:
Maintain accurate and detailed medical records of patient assessments, care provided, and education given.
Supervision and Training:
Supervise and teach other personnel as needed, such as home health aides.
- Clinical Skills:
- Proficient in clinical assessment and the use of medical equipment like blood pressure monitors, glucose meters, and feeding pumps.
Technical Skills:
Experience with Electronic Health Record (EHR) systems.
Soft Skills:
Exceptional communication, time management, organizational, and problem-solving skills, as well as compassion and the ability to work independently.
Education:
A Registered Nurse (RN) license is required, with most positions preferring or requiring a Bachelor of Science in Nursing (BSN) and often some experience.
