Role Description
As a Community Liaison, you will be responsible for building strong relationships with referral sources and generating up to 50 qualified referrals per month. This is a high impact role for a motivated professional with a proven track record in the Home Health industry.

Requirements
- Currently working in the Home Health industry
- Proven ability to consistently deliver high-volume referrals (50 per month)
- Strong network of healthcare contacts
- Excellent communication and relationship-building skills
- Self-motivated and goal oriented
Responsibilities
- Develop and maintain relationships with hospitals, physicians, senior living communities and other referral sources
- Execute strategies to increase patient referrals and grow census
- Represent the agency at networking events and community outreach programs
- Collaborate with internal teams to ensure smooth onboarding of new patients
