Registered Nurse (RN) – Case Manager | California
Position Summary
We are seeking an experienced Registered Nurse (RN) Case Manager to coordinate and manage patient care throughout the healthcare continuum. The Case Manager will collaborate with physicians, interdisciplinary teams, patients, and families to ensure appropriate care planning, resource utilization, discharge planning, and achievement of optimal patient outcomes.
Key Responsibilities:
- Assess, plan, coordinate, and evaluate patient care needs.
- Develop individualized care plans based on patient diagnoses and goals.
- Facilitate discharge planning and coordinate post-acute care services.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals.
- Monitor patient progress and ensure timely interventions.
- Advocate for patients and families to promote quality, cost-effective care.
- Review medical records for medical necessity and appropriate utilization of services.
- Coordinate insurance authorizations and referrals as needed.
- Educate patients and families regarding treatment plans, resources, and follow-up care.
- Maintain accurate and timely documentation in the EMR system.
Requirements:
- Active California RN License.
- BLS Certification required.
- Minimum 1–2 years of Case Management experience preferred.
- Acute care/hospital experience preferred.
- Strong knowledge of discharge planning, utilization review, and care coordination.
- Experience with EMR systems and interdisciplinary collaboration.
- Excellent communication, organizational, and critical-thinking skills.
Preferred Qualifications:
- Case Management certification (CCM, ACM, or equivalent).
- Experience in acute care, medical-surgical, telemetry, or specialty units.
- Familiarity with Medicare, Medicaid, and commercial insurance guidelines.
Salary : $11,000 - $13,000
