Ambassador Personnel is seeking an experienced RN Care Coordinator to support population health initiatives and improve healthcare outcomes across Alabama.
Required Qualifications
- Active Alabama or Compact State RN License
- ADN or higher
- 5 years of healthcare experience
- Experience in care coordination, case management, population health, patient navigation, referral management, or similar programs
- EHR and healthcare technology experience
Important: This role requires care coordination or population health experience beyond traditional bedside nursing.
Preferred Experience
- FQHC, Rural Health Clinic, or primary care
- Chronic disease management and care gap closure
- Patient outreach and referral management
- Value-based care and quality improvement
