Responsibilities
- Provide skilled nursing visits in patients’ homes under RN direction
- Assist with medication management and patient education
- Perform wound care and routine skilled nursing tasks within scope
- Communicate changes in patient condition to RN / clinical team
- Document accurately and on time
- Current Hawaii LPN license
- 1 year clinical experience (home health preferred)
- Strong communication and documentation skills
- Valid driver’s license reliable transportation
- Ability to work independently in the field
