Job Title: Registered Nurse (RN) – Home Health

Reports To: Clinical Supervisor / Director of Nursing

Position Summary:

We are seeking a dedicated and knowledgeable Registered Nurse (RN) to provide skilled home health services. The RN will deliver compassionate, patient-centered care in the home setting, ensuring compliance with Indiana Medicaid regulations and individualized care plans that support health, independence, and quality of life.

Key Responsibilities:

• Deliver skilled nursing services including wound care, medication administration, chronic disease management, and health education.

• Ensure all services are medically necessary and documented to meet Medicaid reimbursement standards.

• Coordinate care with physicians, case managers, and other interdisciplinary team members.

• Educate patients and caregivers on disease processes, medication regimens, and self-care strategies.

• Maintain accurate, timely, and compliant documentation in the electronic health record (EHR).

• Participate in Medicaid audits, quality assurance activities, and case conferences.

• Advocate for patient needs and assist with accessing community resources and support services.

Qualifications:

• Current and unrestricted Registered Nurse (RN) license in the state of Indiana.

• Minimum of 1 year of nursing experience; home health and Medicaid experience strongly preferred.

• Knowledge of Indiana Medicaid policies, documentation standards, and prior authorization processes.

• Current CPR certification.

• Valid driver’s license and reliable transportation.

• Strong clinical judgment, communication, and organizational skills.

• Experience with electronic documentation systems preferred.

Working Conditions:
