Position Overview:
We are seeking an experienced and dynamic Regional Director of Social Services to oversee and support the Social Services departments across our network of skilled nursing and rehabilitation facilities throughout New Jersey.
The Regional Director will provide leadership, guidance, and compliance oversight to ensure that each facility’s Social Services department operates effectively, meets regulatory requirements, and delivers the highest quality of care and resident experience.
Key Responsibilities:
Oversee, train, and support facility-based Directors of Social Services and their teams across multiple sites.
Ensure compliance with all federal, state, and local regulations related to resident rights, discharge planning, and psychosocial care.
Conduct regular audits and on-site visits to assess department performance, identify needs, and implement improvement plans.
Develop and implement standardized policies, procedures, and best practices across facilities.
Collaborate with Administrators, Nursing, Admissions, and interdisciplinary teams to ensure smooth transitions of care and optimal resident outcomes.
Provide consultation for complex cases, including behavioral health and psychosocial issues.
Serve as a liaison with community agencies, hospitals, and external partners.
Support and mentor Social Service staff, promoting professional growth and retention.
Qualifications:
Licensed Social Worker (LSW or LCSW) in New Jersey required.
Bachelor’s or Master’s degree in Social Work or related field.
Minimum of 5 years of experience in long-term care or sub-acute rehabilitation.
Prior regional or multi-facility leadership experience strongly preferred.
Comprehensive understanding of long-term care regulations, discharge planning, and psychosocial documentation.
Excellent communication, leadership, and organizational skills.
Willingness and ability to travel regularly to facilities throughout New Jersey.
