We are seeking an experienced Director of Case Management (RN or Licensed Social Worker preferred) to oversee multiple facilities in New Jersey. This leadership role requires traveling between facilities to provide guidance, ensure high-quality resident care, and maintain regulatory compliance.
Responsibilities:
Provide leadership and oversight of case management operations across multiple facilities.
Travel to each facility regularly to support teams and ensure consistent practices.
Develop and implement policies, procedures, and best practices.
Ensure compliance with state and federal regulations.
Monitor performance metrics, quality outcomes, and resident satisfaction.
Mentor and support case management staff, including RNs and social workers.
Collaborate with clinical and administrative leadership to optimize resident care.
Qualifications:
RN or Licensed Social Worker preferred; proven leadership experience in case management.
Strong knowledge of post-acute, long-term care, and regulatory requirements.
Excellent communication, organizational, and problem-solving skills.
