- Evaluate residents and develop individualized physical therapy treatment plans based on their needs and goals.
- Provide skilled physical therapy services to improve mobility, strength, balance, and overall functional independence.
- Assist residents in achieving their highest possible level of independence and quality of life.
- Collaborate with nursing staff, physicians, occupational therapists, speech-language pathologists, and other healthcare professionals to ensure coordinated resident care.
- Maintain accurate and timely clinical documentation using Net Health.
- Participate in interdisciplinary care planning and resident progress discussions.
- Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, and safety strategies.
- Ensure compliance with all applicable state, federal, and facility regulations.
- Graduate of an accredited Physical Therapy program.
- Current Pennsylvania Physical Therapist (PT) license or eligibility for licensure.
- Previous experience in a SNF/LTC setting is preferred but not required.
- New graduates are encouraged to apply!
- Strong communication, organizational, and interpersonal skills.
- Compassionate, patient-centered approach to care.
- Ability to work independently while contributing to a collaborative rehabilitation team.
- Familiarity with Net Health and PCC is a plus, but not required.
