What You'll Do:
As a Director of Social Services and Admissions, you will carry out our Mission of Love by planning, developing and administering all social services programs within the Health Care Center (while being on-call to Personal Care residents for psychosocial needs). Responsible for census in both the Health Care Center and Personal Care Center. Lead performance improvement projects and provide education and coaching in order to build needed skills in others to lead PIPs. Identify opportunities for improvement through analysis of data, observation of operations, and consultation with leadership and staff.
Our Social Services and Admissions is responsible for the following:
- Oversee that the facility is meeting the social/emotional needs of the residents through participation in MDS evaluations, care planning and the completion of other social services forms of documentation.
- Performs evaluation of each resident's social/emotional needs. This information will be included in the plan of care for each individual resident and will be periodically re-evaluated for current accuracy reflective of the particular person with approaches to be followed to meet those needs identified.

Act as an advocate for resident rights through orienting residents and staff about respecting the rights of residents, education on what constitutes abuse, the reporting and prevention of elder abuse.
- This includes participation in the orientation process for new employees and the annual in-service program.
- Assist in promoting the functioning of the Residents' Council and in helping residents to exercise their rights as citizens, i.e.: registration for voting privileges, transportation to and voting in elections.
- Communicate with government and social agencies for reporting incidents and acquiring needed support services for residents.
- Responsible for discharge planning arrangements and communication to and between various departments, residents and families in coordinating level of care changes and overseeing the processes that takes place during these transitions.
- Assist in the review and revision of Social Services Policies and Procedures and insure the confidentiality of resident social/emotional information.
- Attend meetings and complete projects and tasks as designated by the Executive Director.
- The Director of Social Services and Admissions is responsible for the census of both the Heath Care and Personal Care units. Schedules and performs tours, communicates regularly with potential residents and their families, schedules pre-admission screens for all potential Personal Care residents, communicates with housekeeping and maintenance on room turnovers, etc. while ensuring that both units are at or near full occupancy.

Participate in the admission process of nursing and personal care residents.
- Inform the resident and/or responsible party and secure written acknowledgement from them regarding resident rights and responsibilities as outlined in the Resident Manual and in literature from government agencies.
- Discuss and educate residents and family/responsible party about Living Wills, DNR and other means for control over care.
- Plan and implement residents' discharges from the Masonic Health Care Center to the least restrictive alternate setting (Community, Retirement Living Community or Personal Care Area). Contact and arrange for in-home services when needed.
What You Have:
- Training gained through graduation from a four-year college or university with a degree in social work.
- Personal computer experience would be helpful.
- Good written, oral, organizational, and basic computer skills.
