Job Overview
We are seeking a compassionate and dedicated Hospice Nurse to join our team. In this role, you will provide high-quality nursing care to patients in hospice settings, ensuring comfort and dignity during their end-of-life journey. You will work closely with patients, families, and interdisciplinary teams to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients.
Duties
- Conduct comprehensive patient assessments to determine care needs and preferences.
- Develop and implement personalized care plans in collaboration with the healthcare team.
- Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
- Provide emotional support and education to patients and their families regarding end-of-life care options.
- Monitor patient conditions and report changes to the healthcare team promptly.
- Assist with pain management strategies and acute pain management as needed.
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Collaborate with physicians, social workers, and other healthcare professionals to coordinate care.
- Participate in case management activities to ensure continuity of care across settings.
- Provide guidance on advance directives and facilitate discussions about goals of care.
Skills
Candidates should possess a diverse skill set that includes but is not limited to:
- Proficiency in EMR/EHR systems such as HospiceMD
- Experience in hospice & palliative medicine, geriatrics, or senior care environments.
- Knowledge of medical terminology, anatomy, physiology, and patient assessment techniques.
- Familiarity with ICD coding (ICD-9/ICD-10) and CPT coding for accurate documentation.
- Ability to manage complex cases involving chronic illnesses or developmental disabilities.
- Competence in infection control practices and aseptic technique.
- Experience with various medical procedures such as dialysis, suturing, or botulinum toxin treatment is advantageous.
