We are seeking on-call, base pay plus per-visit LVN Licensed Vocational Nurse or Registered Nurse to join our team in the OC/LA area! You will be responsible for the weekend from 5pm on Friday until 8am on Monday morning. Clinical hospice experience is required and candidates with triage experience are highly desirable.
Responsibilities:
- Provide Skilled Nursing Care in a home or facility setting
- Ensure the appropriate use of meds, DME, and supplies
- Educate and support patients, families, and caregivers
- Participate in IDT updates and updating the Plan of Care
- Provide our patients and families the best care possible
Qualifications:
- Valid LVN or RN License in California
- One year or more of experience in either Home Health or Hospice is required
- Previous experience in palliative care and intake is a plus
- Ability to build rapport with patients
- Compassionate and caring demeanor
- Excellent written and verbal communication skills
Salary : $325 - $1,000
