Position Type: Full Time
Essential Duties & Responsibilities
- Perform comprehensive in-home evaluations of patients’ functional status and living environment
- Develop and implement individualized treatment plans to improve activities of daily living (ADLs)
- Provide therapeutic interventions to enhance strength, coordination, mobility, and cognitive function
- Assess home safety and recommend modifications or adaptive equipment
- Educate patients and caregivers on exercises, safety techniques, and use of assistive devices
- Collaborate with physicians, nurses, and interdisciplinary team members to coordinate care
- Monitor patient progress and adjust treatment plans as needed
- Complete timely and accurate documentation in the electronic medical record (EMR)
- Ensure compliance with regulatory and agency standards
- Valid driver’s license is required.
- Active Massachusetts Occupational Therapist (OT) license is required
- Previous experience as an occupational therapist, preferably in a home health care or outpatient setting.
- Problem-solving skills to modify treatment plans based on patient progress and challenges.
- Excellent communication and interpersonal skills to effectively educate and support patients and their families.
- Ability to work independently and manage patient care in a home setting.
- Proficiency in using electronic health record (EHR) systems for documentation and communication.
- Use of a company vehicle, including paid insurance, maintenance, and gas. (Subject to meeting eligibility requirements.)
