Position Overview

Essential Duties & Responsibilities
- Provide after-hours nursing coverage for hospice patients and families in the home setting
- Triage incoming calls from patients, caregivers, and facilities, prioritizing based on urgency
- Respond promptly to clinical concerns and provide guidance or interventions as appropriate
- Perform emergency and unscheduled home visits when needed
- Assess and manage pain and other end-of-life symptoms using appropriate clinical interventions
- Communicate changes in patient condition to the physician and interdisciplinary team
- Ensure continuity of care by following established care plans and hospice protocols
- Accurately document all patient interactions, assessments, and interventions per agency policy
- Maintain availability and responsiveness during scheduled on-call shifts
- Proficiency in using electronic health record (EHR) systems for documentation and communication
- Participate in hand-off communication with the daytime care team
- Valid driver’s license and reliable transportation
- Active Massachusetts Registered Nurse (RN) license is required
- 1-2 years of recent clinical experience required (hospice, critical care, med‑surg, ICU or ED strongly preferred)
- Strong clinical assessment and symptom‑management skills
- Ability to work independently in the field
