Position Overview
All Care is seeking an RN for the role of VNA Clinical Manager for our new Boston territory. This position is responsible for overseeing a team of clinicians and ensuring the delivery of high-quality patient care in a home health setting. This role manages clinical operations, supports staff performance, and ensures compliance with all regulatory standards while coordinating patient services to achieve optimal health outcomes.
This position follows a Monday–Friday schedule, with four days based in our Lynn office and one day working remotely.

Essential Duties & Responsibilities
- Supervise and support a team of nurses and therapists
- Review, approve, and oversee individualized patient care plans
- Ensure compliance with federal, state, and accreditation standards (e.g., Medicare/Medicaid)
- Monitor clinical documentation for accuracy, timeliness, and completeness
- Conduct case conferences and coordinate care with physicians and interdisciplinary teams
- Track patient outcomes and implement quality improvement initiatives
- Assist with staff hiring, onboarding, training, and performance evaluations
- Manage scheduling and staffing to ensure adequate patient coverage
- Address patient concerns, incidents, and clinical issues promptly
- Attend field visits to patient homes as needed
- Active Massachusetts Registered Nurse (RN) license is required
- Masters of Science in nursing preferred
- 1-2 years of supervisory experience preferred
- 3 years of home health or related clinical experience
- Strong knowledge of home health regulations and reimbursement guidelines
- Proficiency in EMR/EHR systems
- Proficiency in Microsoft Word, Outlook, and Excel is required
