Hospital Liaison

ALIYA Healthcare is seeking an experienced Hospital Liaison to serve as the vital link between Aliya Healthcare facilities and partnering hospitals. This individual will ensure smooth communication, exceptional care coordination, and timely transitions for our residents. The ideal candidate is compassionate, detail-oriented, and highly organized, with a strong understanding of post-acute care and hospital discharge processes.

A minimum of 1 year of experience as a Hospital Liaison is required.

Responsibilities

• The Hospital Liaison will be responsible for following up daily on residents from multiple Aliya Healthcare facilities who are currently in the hospital
• This role ensures that our residents receive the highest standard of care during their hospital stay and facilitates seamless communication between hospital staff, our facilities, residents, and their families
• Conduct daily visits to hospitals where our residents are admitted
• Monitor the condition and progress of hospitalized residents
• Communicate regularly with hospital staff to obtain updates on residents' health status and treatment plans
• Ensure that all necessary information is accurately communicated back to the respective nursing home facilities
• Serve as a point of contact for families of hospitalized residents, providing updates and addressing any concerns
• Coordinate with Aliya Facility staff to prepare for residents' return, ensuring a smooth transition back to the facility
• Advocate for residents' needs and preferences during their hospital stay
• Collaborate with hospital discharge planners and case managers for discharge plans
• Maintain records of all hospital visits, interactions, and follow-ups
• Attend meetings with Aliya Healthcare staff to report on hospitalized residents' statuses and discuss any issues or concerns
