Role Description
This is a full-time, on-site role for a Hospital Liaison based in the Greater Chicago Area. The Hospital Liaison will act as a critical point of communication between hospitals, patients, and Serenity Estates. Responsibilities include coordinating patient referrals, facilitating smooth transitions from hospital care to our facilities, and fostering relationships with healthcare professionals to ensure seamless patient care. Additionally, the role involves representing Serenity Estates during meetings and events to bolster partnerships and streamline discharge planning for exemplary patient outcomes.

- Experience in patient coordination, discharge planning, and healthcare referrals
- Familiarity with healthcare systems, services, and processes in hospital settings
