Ahava Therapy Services is hiring a Full-Time OT for Franklin Wellness & Rehab, an 83-bed skilled nursing facility that provides both short- and long-term care.

Therapists at Franklin work as part of a collaborative in-house rehab team, with strong leadership support, to help residents achieve independence and optimal function.

It’s also steeped in Civil War history, with sites like the Carnton Plantation and the Carter House. Franklin offers residents a dense suburban feel. Plus, it’s just a short drive from Nashville, making it a popular spot for those who love a mix of past and present.

⚙️ Responsibilities

• Review clinical information and evaluate its impact on the assessment and treatment process.
• Conduct comprehensive functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable functional goals.
• Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly.
• Revise treatment plans based on patient performance and progress.
• Supervise and guide therapy assistants, ensuring proper treatment techniques and the delivery of care.
• Educate residents, families, and caregivers about exercises and therapeutic procedures to support carryover.
• Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes.
• Assist with discharge planning and maintain knowledge of community resources.
• Participate in care conferences, meetings, and quality improvement initiatives.
• Maintain clinical competence through continuing education opportunities.
• Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines.
• Support the company’s mission, vision, and professional expectations.
• Maintain expected productivity levels and demonstrate excellent teamwork and communication skills.
  
  
  

✔️ Qualifications

• Bachelor’s or Master’s Degree in Occupational Therapy
• Active OT license in the state of practice
• BLS/CPR certification (current or obtained during orientation)
• Strong clinical reasoning, communication, and documentation skills
• Ability to collaborate effectively within an interdisciplinary team
