Cordova Wellness & Rehab Center is a 242-bed skilled nursing facility offering short-term rehab, long-term care, and outpatient therapy services.

Join a supportive, in-house therapy team under outstanding leadership, with a strong focus on collaboration, functional progress, and meaningful clinical impact.

⚙️ Responsibilities

• Review clinical information and evaluate how it impacts the assessment and treatment process.
• Conduct comprehensive functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable functional goals.
• Document evaluations, daily treatments, progress notes, and discharge summaries accurately and timely.
• Revise treatment plans based on patient performance and progress.
• Supervise and guide therapy assistants, ensuring proper treatment techniques and care delivery.
• Educate residents, families, and caregivers on exercises and therapeutic procedures for carryover.
• Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes.
• Assist with discharge planning and maintain knowledge of community resources.
• Participate in care conferences, meetings, and quality improvement initiatives.
• Maintain clinical competence through continuing education opportunities.
• Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines.
• Support the company’s mission, vision, and professional expectations.
• Maintain expected productivity levels and demonstrate excellent teamwork and communication skills.
  
  
  

✔️ Qualifications

• Bachelor’s or Master’s Degree in Occupational Therapy
• Active Tennessee OT license
• BLS/CPR certification (current or obtained during orientation)
• Strong clinical reasoning, communication, and documentation skills
• Ability to collaborate effectively within an interdisciplinary team
