RN Admissions Nurse – Therapeutic Interchange | Skilled Nursing

The Agawam RehabNursing Campus is seeking an RN Admissions Nurse - Therapeutic Interchange to join our team in a full-time role. This position plays an important role in ensuring a smooth admissions process for new residents while overseeing the facility's therapeutic interchange program.

The RN Admissions Nurse will initiate admission assessments for new residents and assist with administrative tasks such as collecting medical information and ensuring all insurance documentation is completed. This role is also responsible for coordinating the therapeutic interchange program, which allows approved medication substitutions for residents when appropriate.

Key Responsibilities

• Initiate admission assessments for new residents entering the facility

• Assist with the admissions process to ensure a positive experience for residents, families, and staff

• Respond to inquiries regarding potential residents in a professional and compassionate manner

• Obtain resident insurance information when referrals are received

• Meet with residents and families upon admission to ensure all required forms are completed

• Maintain admission guidelines by assisting with updates to admission criteria, policies, and procedures

• Support residents and families with questions related to admissions and the intake process

• Organize and enter resident information into facility databases

• Handle phone calls and emails from prospective residents and referral sources

• Oversee the therapeutic interchange program and coordinate approved medication substitutions in collaboration with the pharmacy and medical team

Qualifications

• Active RN license required

• Experience in post-acute care, skilled nursing, or rehabilitation preferred

• Strong communication and organizational skills

• Ability to manage admissions documentation and resident information accurately
