- Schedule: Full-Time
- Setting: Field-Based / Home Care
- Patient Population: Pediatrics & Geriatrics
- Territory: Fairfield, Litchfield, Hartford & New Haven Counties
- Provide and oversee high-quality in-home nursing care
- Develop and manage individualized care plans
- Coordinate with physicians and interdisciplinary care teams
- Monitor, document, and report changes in patient condition
- Supervise and support home care staff
- Ensure patient safety, comfort, and independence at home
- Active Connecticut RN license
- Minimum 2 years RN experience (acute care required; home care preferred)
- Experience with pediatric and geriatric patients
- Clinical skills in:
- Ventilator and tracheostomy care
- G-tube/PEG tube management
- IV infusions and wound care
- Current BLS certification
- Valid driver’s license
- Authorized to work in the U.S.
