The Private Duty LPN/RN provides skilled nursing care to assigned patients, following the nursing process of assessment, planning, implementation, and evaluation while adhering to relevant laws, policies, and standards.
Key Responsibilities
- Administer ordered treatments and medications per care plan and physician's instructions.
- Identify and address new problems, notifying supervisors and physicians as needed, and document all actions.
- Perform clinical observations, communicate significant changes, and request order modifications based on patient status.
- Work collaboratively as part of a team under RN supervision, ensuring accurate documentation and appropriate use of services.
- Participate in mandated activities, adhere to policies, and maintain professional appearance and punctuality.
- Graduation from an accredited program and valid licensure to practice.
- Relevant professional experience and successful competency testing.
- Reliable transportation, current TB and CPR certifications.
- Organized, flexible, with sound judgment and physical ability to perform duties.
- Ability to perform physical activities including lifting and movement, with vision and hearing within normal limits.
- Work occurs mainly in private homes with variable conditions, potential exposure to hazards, and frequent travel required.
