- Assess, diagnose, and oversee primary care for patients
- Prescribe medications and manage treatment plans
- Maintain electronic medical records using Canopy and Greenway
- Collaborate with regional and center leadership to meet quality goals
- Work with practice managers to direct care teams
- Perform other duties as assigned
- MD or DO graduate with Internal Medicine or Family Medicine certification (or eligibility)
- Active, unrestricted state license and DEA license
- US work authorization
- Passionate about providing exceptional care to older adults
- Geriatrics fellowship or experience
- Experience in value-based healthcare and outpatient primary care
- Bilingual abilities, if applicable
