Role: Primary Care Physician responsible for delivering equitable, value-based healthcare to Medicare patients in a neighborhood clinic setting.
Responsibilities: Assess and diagnose patients, oversee primary care, prescribe treatments, maintain electronic medical records, collaborate with leadership to meet quality goals, manage care teams, and perform additional duties as needed.
Qualifications: MD or DO with board certification or eligibility in Internal Medicine or Family Medicine, active unrestricted license, DEA license, and US work authorization. Preferred: Geriatrics fellowship, geriatric experience, outpatient primary care, experience in value-based models, bilingual skills.
