Licensed Practical Nurse (LPN) Pediatric Home Health
Role Overview: Provide focused, one-on-one pediatric home health nursing for children with complex needs, including tracheostomy and ventilator dependence. Collaborate closely with registered nurses and clinical leadership to deliver safe, consistent care in patients' homes.
Responsibilities: Support skilled nursing needs, follow plans of care, and maintain patient relationships. Utilize the Employee Portal for shift management, request support, and stay connected. Ensure safety with PPE and CDC guidelines.
Qualifications & Skills: Valid NJ LPN license, recent physical, PPD or chest X-ray, BLS CPR, valid driver’s license, and experience or willingness to train in G-tube, trach, or vent care.
