Seeking a Physical Therapist (PT) to join a home health care team, enabling patients to recover at home. Full-time weekly visits required for Osceola County.
Key Responsibilities
- Conduct initial and ongoing assessments of mobility, strength, and function.
- Administer and interpret tests to set functional goals and outcomes.
- Provide written evaluations and care plans outlining needs, goals, and progress.
- Deliver therapy services per physician orders, documenting performance and modifying plans as needed.
- Manage discharge planning and educate patients and caregivers on home exercises.
- Coordinate with healthcare team, including physicians and family members.
- Ensure accurate documentation to prevent claim denials.
- Supervise personnel (PTA, HHA) as applicable and complete assessments where permitted.
- Degree in Physical Therapy from an accredited program.
- Current, unrestricted state license.
- Valid CPR, driver’s license, and proof of insurance.
