The Field Supervisor & Coordinator will help clients and employees in the territory onboard. The Field Supervisor & Coordinator will
- Educate clients about their rights and terms of their services.
- Onboard new or transfer clients and conduct their intakes.
- Onboard and supervise employees in the territory.
- Effectively speak and translate for/with Spanish speakers during interaction with office.
- Conduct supervisory field visits and/or trainings.
- Create plan of care for new clients.
- Supervise assigned caregivers (CNA'S, PCA'S and HHA'S) to assure performance of tasks, functions and responsibilities necessary to assure appropriate individual care plan and active treatment is offered in a clean, safe and secure environment, in accordance with federal, state and unit policies and procedures.
- Provide supervision, coaching and evaluation of caregivers (Caregivers, PCAs,DCWs, and HHAs) on assigned case.
- Comfortable working on the computer and communicating in writen form
- Comfortable to objectively document and complete timely incident reports and EIM
- Work directly with our head of marketing and Care Coordinator manager
- Ability to work in a dynamic team
- Ability to work independently
- Maintain an audit ready, fraud free operation
- Ability to manage DOH, Waiver, MCO audits
- 3 full years of caregiving experience required, Home Health Aide training, Adult Residential License, Direct Care worker certification and/or CNA license required.
- 3 full years experience in a Supervisory, Managerial role
- Display tact, good judgement, interest in people and maturity in dealing with the frail, ill population.
- Display flexibility, cooperation and dependability in fulfilling responsibilities
- Reliable and professional conduct
- Bilingual English and Spanish speaking staff who is able to communicate effectively to Spanish and English staff and clients is required.
- Comfort with facilitating and supporting nonSpanish english staff is mandatory. .
- Driver’s License
- Comfortable with Driving
- Assist with client scheduling to support Care coordinator to ensure safe cover
