What You'll Do:
- Be a key player in ensuring our residents receive holistic, person-centered care
- Lead efforts to address the psychological, social, and physical needs of residents and their loved ones
- Collaborate with nursing, therapy, and administrative teams to create a true community of care
- Complete social assessments and gather important information at admission
- Maintain ongoing communication and support with residents' families
- Assist our Admissions Coordinator by giving thoughtful, welcoming tours to prospective residents and families
- Complete pre-admission assessments to help residents transition smoothly into our facility
- A degree from an accredited school of social work
- Licensure in the state of practice (LSW, LCSW, or LICSW required)
- 2 years of experience in long-term care preferred
