Position: Licensed Practical Nurse
Pay: $30-$40/hr on W2.

Job Description:
The role involves providing comprehensive health assessments and completing MDS-HC (Minimum Data Set – Home Care) evaluations to accurately evaluate each member’s health condition and care needs.
The candidate will be responsible for conducting Adult Foster Care visits and performing Home Care visits, with reporting duties based at the office.
Key responsibilities include maintaining detailed documentation of each member’s health status, promptly reporting any significant changes to supervising nurses or healthcare providers, and collaborating with an interdisciplinary team to develop and update individualized care plans that meet each member’s evolving needs.
The position also requires offering guidance and supervision to Caregivers and direct care staff to ensure the consistent delivery of safe, high-quality services.
The candidate must adhere to Massachusetts Department of Public Health regulations and Adult Foster Care (AFC) program standards, while promoting wellness, safety, and independence through health education and counseling for members and caregivers.
Participation in emergency preparedness activities and maintaining safety standards within the home care setting are also essential.
Additionally, the role involves maintaining accurate, up-to-date clinical documentation, including monthly nursing assessments, progress notes, and MDS-HC records.
Pay: $25.00 - $40.00 per hour
