Key Responsibilities
- Provide direct nursing services to patients in home and community environments.
- Work collaboratively with the Nursing Supervisor and primary nurse to implement individualized care plans.
- Perform patient assessments, monitoring, and documentation of care provided.
- Support patients and families with education and guidance related to healthcare needs.
- Maintain accurate documentation and comply with privacy and confidentiality standards.
- Communicate effectively with patients, caregivers, and healthcare team members to ensure quality care.
Education
- Graduate of an approved Practical or Vocational Nursing program.
- Minimum of two years of medical or surgical nursing experience strongly preferred.
- Knowledge and experience in community health nursing preferred.
- Home care or home health experience preferred.
- Current Massachusetts Licensed Practical Nurse license.
- Basic Life Support certification for Healthcare Providers required.
- Strong fundamental nursing skills.
- Effective communication and interpersonal abilities.
- Patient assessment and critical thinking skills.
- Strong organizational and time management skills.
- Proficiency with computer systems and electronic documentation.
- Ability to maintain confidential patient information.
- Ability to read, write, and communicate effectively in English.
- Excellent customer service skills.
