As a Case Manager, you will be responsible for providing comprehensive case management services to individuals in need. This role involves assessing clients' needs, developing care plans, and coordinating services to support their well-being and progress.
Responsibilities
- Conduct intake assessments and develop individualized care plans
- Provide intervention and support to clients in need - Utilize motivational interviewing techniques to assess client needs and goals - Coordinate services and resources for clients - Conduct Psychosocial/financials assessments and develop individualized care plans - Mentor and supervise clients in various life skills - Manage situations effectively with a focus on de-escalation
- Monitor client progress and adjust care plans as needed
