What are the responsibilities and job description for the Care Navigator position at Adobe Care And Wellness Arizona LLC?
SUMMARY: Adobe Care and Wellness is a case management team providing quality health care for counties all over Arizona. Centers of excellence include integrated case management, in home/in clinic wellness assessments, preventative care, transitional care, and providing social work services. We are an integrated health solutions company which specializes in customizing programs for insurance groups, providers, hospitals, and families.
DUTIES AND RESPONSIBILITIES:
The care navigator is responsible for conducting a transitional questionnaire for our high-risk members to determine risk factors in the members’ homes
Participate in finding solutions for the presenting problem working collaboratively with the member and Case Managers
Determine whether the member needs an LPN Case Manager, Social Worker, or Registered Dietitian
Perform preventative screenings such as HbA1c checks, Colorectal Fit-kits, Microalbumin checks, and Peripheral Artery Disease test
Visit member’s homes alone to perform assessments and screenings
Compliance with HIPAA
Performs other related duties as assigned by management.
