Position Type: Full-Time, Monday-Friday (8 AM - 5 PM, flexible schedule)
Compensation: $54-$89 per hour / 35-40 hours per week
Job Overview
A home health organization is seeking experienced Registered Nurses to provide comprehensive assessments and skilled nursing care in the Eugene, OR area. The RN will deliver high-quality patient care in the home setting, including wound management, IV therapy, and chronic disease support. This role involves educating patients and families, coordinating with an interdisciplinary team, and managing an evolving caseload. A $5k sign-on bonus is available.
Responsibilities

Conduct comprehensive nursing assessments to evaluate patient and family needs in the home setting.

Develop, implement, and regularly update individualized patient care plans in collaboration with patients, families, and the care team.

Provide skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.

Educate patients and caregivers on disease processes, medications, treatment plans, and self-care strategies.

Coordinate care with physicians, therapists, social workers, and other interdisciplinary team members to ensure continuity of care.

Monitor patient progress, document clinical findings, and update EMR records within required timeframes.

Identify and plan for discharge needs to ensure smooth transitions of care.
Qualifications
Required:

Active Oregon Registered Nurse (RN) license

Current hands-on BLS/CPR certification

Reliable transportation with valid driver’s license and insurance

Minimum 1 year of recent acute care or home health experience

Strong clinical judgment and critical-thinking skills

Excellent communication, documentation, and patient-education abilities

Ability to work independently and manage a dynamic caseload

Authorized to work in the U.S.
Preferred:

Associate’s or Bachelor’s Degree in Nursing (BSN preferred)

Home health experience strongly preferred

Day-to-Day Activities

Review daily schedule and plan patient visits to maximize efficiency

Perform on-site assessments and provide skilled nursing interventions (wound care, medication management, IV therapy)

Document visits in the EMR between appointments

Coordinate with physicians, therapists, social workers, and case managers as needed

Team & Collaboration
This role reports directly to the Clinical Manager of Home Health Services, with collaboration alongside regional therapy leadership. You will join an interdisciplinary team of clinicians—including PTs, OTs, nurses, and social workers—and work closely with a PT mentor, case managers, and rehab aides.
