The Adams County Health Department Nursing Division is comprised of Nurse Family Partnership, Nurse Support Program, Harm Reduction, Sexual Health, Immunizations, Child Maltreatment and Fatality Prevention, and Care Coordination for Children and Youth with Special Health Care Needs. The Nursing Division is made up of clinical support staff, medical assistants, harm reduction specialists, operations support, nurses, advanced practice nurses, and midwives.

The Program Manager for ACHD’s nurse home visitation program Nurse-Family Partnership (NFP) provides program oversight, nursing leadership and supervision for public health nurses and program specialists within the program. The position is responsible for budget monitoring, policy and procedure standards, program reporting to funders, performance evaluation of direct reports and program operations. Recommends and administers actions impacting employee pay and status, hiring and discipline. Serves as a resource or expert in program areas providing technical guidance and education to assure program success.

NFP is a research-based program that has been demonstrated to significantly improve critical outcomes for eligible women and their children who receive services in local communities. One primary purpose of the role is to work within the local community and lead agency structure to assure that the program is implemented in accordance with established NFP Visit-to-Visit Guidelines and contract requirements. Another primary purpose for this position is using principles of supervision, reflective practice, and staff development, to provide primary support and appropriate oversight to nurse home visitors, and to build and model a strengths-based, client-centered culture.

Examples of Duties for Success

  • Effectively provide direct supervision for program staff, including nurse home visitors, program support specialists and students.
  • Responsible for monitoring compliance with program contracts, budgets, expenditures, and billing processes of the NFP program.
  • Assure robust staff development, including orientation and training opportunities for maximum program delivery.
  • Assure staff compliance with ACHD guidelines, objectives, procedures, and protocols.
  • Ensure that staff complete all required Nurse-Family Partnership Education sessions as outlined in the orientation materials and agreement with the National Service Office.
  • Develop and maintain active working knowledge of the principles, practices, techniques, and protocols related to community health nursing and its application in public health programs.
  • Oversee and ensure timely and accurate data input and utilization of the reports available through Nurse-Family Partnership.
  • Read and interpret reports; share appropriately with ACHD leadership, team, and partners.
  • Ensure implementation of quality improvement and strategies.
  • Develop methods for ongoing assessment of NFP staff development needs; compare available data to structure appropriate continuing education and guidance of staff.
  • Provide routine surveillance of potential NFP client care incidents.
  • Evaluate implementing agency outcomes and interventions related to NFP.
  • Develop and maintain active working knowledge of inter-related professional services and resources available through ACHD and the community for implementation of programs.
  • Actively participate in local, state and community boards and other community outreach activities to promote understanding and support of public health nursing programs and to promote local, state, and national standards.
  • Collaborate with professional and community groups to develop goals and increase access to public health care programs.
  • Coordinate and integrate program activities with other nursing programs, ACHD divisions, allied agencies, service providers, and community groups.
  • Provide consultation, leadership, and management in specialized program areas.
  • Provide technical expertise to ACHD staff in the interpretation of program policy, procedures, and protocols. Participates in nursing, emergency preparedness and other program staff team conferences to provide expertise and coordination of activities.
  • Actively participate in various committees and boards related to program activities and responsibilities. Engage in specialized programs, projects, and research in the field of public health nursing.
  • Perform other duties as assigned.
Additional Duties:
  • Assist in the orientation and training of new employees, students, and interns.
  • Participate in quality improvement projects.
  • Maintain electronic health record (EHR) proficiency and skills as necessary for daily duties.

Qualifications for Success

Knowledge


  • Standards of Nursing Practice, Nurse Practice Act, and national nursing regulations.
  • State and federal rules, including HIPAA and mandatory reporting.
  • Policies and procedures relevant to NFP home visiting; ability to establish new policies as needed.
  • Maternal and child health theory and public/community health nursing practices.
  • Prevention models, public health programs, and population health concepts, including social determinants of health.
  • NFP program principles, theories, and implementation.

Working Conditions and Physical Requirements:
  • This position spends approximately 60% time sitting, driving, and 40% standing, and/or walking.
  • Utilize appropriate ergonomics.
  • Occasionally lifts, carries, pulls, or pushes up to 20 lbs.
  • Occasionally uses cart, dolly, or other equipment to carry in excess of 20 lbs.
  • Occasionally uses stairs, climbs, stoops, kneels, balances, reaches, crawls and crouches while performing office or work duties.
  • Verbal and auditory capacity enabling constant interpersonal communication through automated devices, such as telephones, radios, and similar; and in public meetings and personal interactions.
  • Constant use of eye, hand and finger coordination enabling the use of automated office machinery or equipment.
  • Visual capacity enabling constant use of electronic devices or other work-related equipment.
Emergency Preparedness Duties:
  • Employee responds, as required, to support public health emergencies, incidents, and events.
  • Employee participates in all exercises and drills on emergency preparedness, as required.
  • Completes trainings identified as appropriate for this level employee.
  • This may require weekend, evening, and on-call duties.
Equipment Used:
  • Personal vehicle in good working condition for travel to home visits and community meetings.
  • Must demonstrate proficiency in using personal protective equipment (PPE).
  • Electronic device and computer use including cell phone, internet, email, Microsoft Office Suite.
  • Audio-visual and other teaching equipment.
  • Healthcare equipment.
  • Must be able to learn and become proficient with equipment and software programs as required to perform assigned duties effectively and efficiently.

More Qualifications for Success

Education and Experience:


  • Bachelor’s degree in nursing required
  • Master’s degree in nursing or closely related field preferred
  • Requires at least five years in program management related work or similar field, with three years of progressive capacity, supervision, or leadership experience.
  • Licensed to practice as a registered nurse (RN) in the State of Colorado or a Compact State Required
Professional Qualifications:
  • Current BLS certification
  • Demonstrated experience in Reflective Supervision, Motivational Interviewing preferred.
Other Qualifications:
  • Flexibility in work hours may be required to accommodate community events, meetings, and outreach activities.
  • Travel within the community and participation in evening, weekend and on call duties may be required.
  • This position requires a valid driver’s license and reliable transportation.
